There are three ways to sign up for CarrierLink. Look over the options shown below and select the one you find most convenient. If you choose one of the first two options, click the corresponding "Select" button. The third option requires only a phone call and an application will be mailed to you.

All options require that you complete an application form and sign a separate release form. These forms are mailed to the CarrierLink office after which you will receive notice of your participation in the program.

1. Fill Out a Form on Your Computer, Print It, Sign It, Mail It

When you select this option, here is what happens:

  • You will be presented with the application form to fill out on your computer screen.

  • This form provides:

    • a Review button which checks your form for errors and if error free...

    • a Print Preview button which will show a "printer friendly" application form and...

    • a Print button which will print the application and release forms.

      Complete instructions can be found at the bottom of the on-screen application form.

  • Sign the release form in the space provided and mail the application (two pages) to the address shown on the form.

2. Print a Form, Fill It Out by Hand, Sign It, Mail It

This option provides a blank application and release form:

  • When you click the "Select" button for this option, "printer friendly" versions of the application and release forms appear on the screen. You will note this version has empty lines and spaces for you to fill out by hand. A click on the "Print" button will print the forms.

  • Fill out the application form, read and sign the release form in the space provided, and mail both forms (two pages) to the address shown on the form.

3. Contact CarrierLink by Telephone

If neither of the first two options suits you (e.g. both require your computer to have a printer), you may call the Volunteer Office at Providence St. Vincent Medical Center at 503-216-2162 and ask that the application forms be sent to you by mail. After you fill out and sign the forms, you will mail them back to the CarrierLink office.


CONFIDENTIALITY STATEMENT: The information you provide on the CarrierLink application form is considered confidential and will be used ONLY if a potential emergency arises. Information you enter on this web site to aid your preparation of the CarrierLink application is NOT shared, sold, or retained for any other use. It is deleted immediately upon your exiting the CarrierLink web site.
